Creating/Using Forms Builder Templates

The Forms Builder Template tool lets you create reusable forms that you can import into your Events and then use them within the Events to collect responses from team members, clients, attendees, etc.

To Create a New Form Template from Scratch:

  1. Click on "Templates" in the main navigation bar and click on the "Forms Builder Templates" link.
  2. Click on the "Add Template" button.
  3. Enter the Form Template name.
  4. Designate if you want your organization's logo and name included at the top of the form.
  5. Designate if you want to include fields at the top of the form to collect the following information from respondents:
    • IMPORTANT NOTE: If you want your respondents to be able to return to this form to make edits/changes to their responses, you MUST capture their email address because it will be used to identify them when they return.
    • Full name
    • Email address
    • Phone
    • Address
  6. Click "Save".

This brings you to the Form Dashboard where you can start adding Questions and Items to your Form. To add a Question/Item:

  1. Click on the "Add Form Item" button.
  2. Enter the Question text or item label.
  3. Select one of the following Question/Item types:
    • Short Answer (short blank answer field)
    • Long Answer (long blank answer field)
    • Multiple Choice - Only One Answer (radio buttons)
    • Multiple Choice - Only One Answer (dropdown menu)
    • Multiple Choice - Multiple Answers (checkboxes)
    • Descriptive Text (no answers)
    • Image with Label (no answers)
  4. Select if the question is optional or required.
  5. Click "Continue".
  6. FOR ALL MULTIPLE CHOICE QUESTIONS:
    • Enter the first Answer Option and click "Save This Option".
    • Repeat for additional Answer Options until all options have been added.
    • Use the up/down arrow next to each item to rearrange the items in the desired order.
    • Click "Done".
  7. FOR DESCRIPTIVE TEXT ITEM:
    • Enter the note/text in the text box and click "Continue" when finished.
  8. FOR IMAGE WITH LABEL ITEM:
    • Enter the Question text or item label.
    • Select an image file from your computer to upload.

To Create a Form Template from an Existing Form:

  1. Click on the "Forms Builder" tool button in the Event Dashboard.
  2. Click on the "Import/Export" button and select the "Save as Template" option.
  3. From the dropdown menu, select the form that you wish to save as a Template.
  4. After clicking "Save", the selected form has been saved as a new Form Template in the Templates area, which you can now edit and modify.

To Import a Form Template into a New Form:

  1. Click on the "Forms Builder" tool button in the Event Dashboard.
  2. Click on the "Import/Export" button and select the "Import Template as New Form" option.
  3. From the dropdown menu, select the Form Template that you wish to import as a new form.
  4. After clicking "Save", the selected Form Template has been saved as a new form. It will include all details from the Template (including the name), which you can now edit and modify.
