Adding/Managing Questions/Form Items

You can add a variety of question types and form items to your forms:

• Question Types
• Short answer
• Long answer
• Multiple choice - only one answer (radio buttons)
• Multiple choice - only one answer (dropdown menu)
• Multiple choice - multiple answers (checkboxes)
• Other Form Items
• Descriptive Text (no answers)
• Image w/ Label (no answers)

You can make any question optional or required for the respondents, and for multiple choice questions you can add an unlimited number of Answer Options.

To Add a Questions/Form Items to a Form:

1. Click on the "Forms Builder" tool button on the Event or Lead Dashboard - OR - use the Quick Launch or Switch Event tool to select the appropriate Event and the "Forms Builder" tool.
2. Find the appropriate Form and click on it.
3. Click on the "Add New Question/Item" button.
4. Select one of the following Question/Item types:
• Short Answer (short blank answer field)
• Long Answer (long blank answer field)
• Multiple Choice - Only One Answer (radio buttons)
• Multiple Choice - Only One Answer (dropdown menu)
• Multiple Choice - Multiple Answers (checkboxes)
• Descriptive Text (no answers)
• Image with Label (no answers)
5. Select if the question is optional or required.
6. Click "Continue".
7. FOR ALL MULTIPLE CHOICE QUESTIONS:
• Enter the first Answer Option and click "Save This Option".
• Repeat for additional Answer Options until all options have been added.
• Use the up/down arrow next to each item to rearrange the items in the desired order.
• Click "Done".
8. FOR DESCRIPTIVE TEXT ITEM:
• Enter the note/text in the text box and click "Continue" when finished.
9. FOR IMAGE WITH LABEL ITEM:
• Enter the Question text or item label.
• Select an image file from your computer to upload.

Once you have added a question, you can make revisions to it by clicking on the "Edit" button under the question. You can also move the item by clicking on the "Move" button under the item as well as delete the item by clicking on the "Delete" button.

Finally, you can clear all the responses for a question by clicking on the "Clear Responses" button. WARNING: This will delete all responses for the question, and this data cannot be retrieved.
